Getting Started

This form is intended to be completed by a pastor or church clerk, though any designated person from your church can fill it in. You may also share your unique “Save and Continue Later” link with someone else who can help with various sections.

You can move between pages using the Next and Previous buttons, or by the page links at the top.

As you complete this form, please answer as accurately as possible.

On the final confirmation page, you will have the option to receive a copy of your responses by email.

Pre-filling Answers from Last Year

If your church submitted a report last year and you have the report code, the form can pre-fill some of the information to save you time. You can always edit the information if something has changed. (Please note that any fields that were left blank in the previous report will not pre-fill.)

To find your report code, retrieve the report from last year. The report code can be found at the bottom of the email you receive.

Automatic Saving

• Logged in users – If you are logged in to the CBAC website, your progress will automatically be saved each time you move to a new page. You will be able to return to the form page and continue working on the draft until you submit the report.
• Guest users – If you do not have a CBAC user account or are not logged in, the first time you visit the form, you will be prompted to choose whether or not you would like to save your progress as you go. Choosing ‘Yes’ will save the form responses in your browser session for up to 30 days. If you are using a shared device or public computer, this information will be accessible to others, and you should choose ‘No’. You will still have the option to “Save and Continue Later” if needed.
• It is not necessary to save your progress as you move from page to page if you are logged in, choose ‘Yes’ as described above, or if you complete the report in one session.

Please note that saving your progress is NOT the same as submitting your form. Your report will not be received until it is submitted.

Save and Continue Later

• The Save and Continue Later button is found at the bottom of the form page. Clicking this button will save your progress and send you an email with a link to continue the form within the next 30 days. The link can also be shared with someone else to collaborate on the report responses.
• Link Recovery – If you lose the link to your report draft, we may be able to recover it for you, although there are a number of reasons why that may not be possible. Please contact [email protected] with the approximate date and time you were working on the report.
